Health of Suffolk County Hospitals?

How will our hospitals do with repeal of the ACA? As reported in the East Hampton Press, repeal of Obamacare is predicted to hurt local Suffolk hospitals. About 150,000 people in Suffolk County are at risk of losing health insurance. Robert Chaloner, CEO of Southampton Hospital predicts a sharp rise of uninsured people, crowded ERs, longer wait times, and of course less federal dollars to offset losses. NY State is a Medicaid expansion state. Such states have seen a 34% reduction in uncompensated care costs in clear distinction to those states that did not accept Medicaid expansion (Kaiser CMU analysis of the Medicare Cost Reports 2013/2014). This will disappear with repeal of the ACA.

What does this mean for our local hospitals? I took a look at the financials (Annual Reports) of 9 of them. Together they employ 17,000 people and their combined yearly revenue is $4,719,000,000 (2015) most of which goes into employee salaries and therefore raises the standard of living and the economy in Suffolk County. Four of these hospitals (Peconic Bay, Brookhaven, Stony Brook and Southside in Bay Shore) are designated “safety net hospitals” as they serve over 40% of out-patients (and over 35% of in-patients) that are either on Medicaid, or on Medicare/Medicaid, or are uninsured. These hospitals are most likely to suffer big time. By comparison, Robert Chaloner, quoted in last week’s report, and Southampton Hospital might have it easy, as their profit margin is still relatively high (7.5% in 2014). I note that financial leverage (liabilities/assets) is high among several Suffolk county hospitals ranging from 0.6 to 1.2 (while the national average is 0.46 for non-profit general hospitals). This too represents a risk.

In NY City 16 hospitals have closed since 2003 and another dozen are in precarious financial condition. Throughout the State, there are hospitals that are teetering. They are often “safety net hospitals”. According to New York’s Department of Health Commissioner, “nearly half [of] New York’s 227 hospitals are financially distressed.”

The brewing storm after repeal of the ACA will likely dwarf the many complaints about Obamacare. Just listen to Congressman Zeldin who can’t stop complaining, but has not provided any constructive solutions to save our hospitals and the related local medical economy, which includes salaries for hospital health-care workers, including doctors.

Repealing the ACA

Lots of promises are being made about a replacement for Obamacare (Affordable Care Act). These include sound bites like “care for all” and “less cost”.

Let’s look at a few realities of this extremely complex issue.

  • By insuring people with preexisting conditions and expanding family plan coverage to 26-year-olds, we are increasing costs to the insurance companies. It should not come as a surprise that premiums are going to rise.
  • For some, the requirement to purchase insurance or pay a penalty may seem like too much government oversight. What do you think happens when these uninsured individuals get sick? They come to hospital emergency rooms who must provide care. This care is paid for mainly by federal, state and local governments; in other words, you and I. This is like someone driving a car without insurance and taxpayers chipping in to pay their car repairs and medical bills.
  • No one can promise you that you can keep your doctor. Physicians and/or the health systems that employ them determine which insurance they will accept.
  • There are lesser known aspects of the Affordable Care Act. Will they be included in future legislation? These include:
    • Regulations to protect the privacy and security of our health information;
    • Incentives for hospitals and physician offices to implement electronic health records;
    • The requirement for sharing data from these electronic records between health organizations to facilitate care;
    • The creation of patient portals so that we can have access to our vital information such as medication lists, lab results and diagnoses;
    • Use of this data to improve the quality of care and to eventually drive how your doctor is compensated.

We are seeing many recent examples where a rush to implement a campaign promise has resulted in unintended and disastrous consequences. Let’s not let this happen to our healthcare system.

‘Mexico City’ Policy

‘Mexico City’ Policy has been reinstated. I spent much of my early working
years in DC at a women’s organization battling for abortion funding for
non-governmental organizations (NGOs) thanks to this restrictive Reagan
policy. It was rewarding work.

NGOs are the backbone to many social programs in poor countries. They fill
a gap that the governments can’t or won’t. Often at great risk to the NGO
workers who go into war torn and poor rural areas to help the population.
I saw first hand the effects of denying women in 3rd world countries
access to abortions in cases of rape, incest and sheer poverty.  These are
often young girls and women who can’t feed the children they have, due to
extreme poverty, thanks to a myriad of reasons including war,
economic/social inequality and political strife. Birth control isn’t
always readily available or culturally acceptable.
These women are victims. The NGOs provide healthcare for these families
and often a safe abortion is the only way for the women to survive –
emotionally and/or physically.  And if not done by doctors through an NGO,
the women will often resort to less safe ways to abort resulting in death
or serious illness. You know the scenario.

Obama rescinded Mexico City in 2009. Today Donald Trump reinstated it.
Bottom line – if you are an NGO and provideor even mention abortion services to women in third world countries you will lose US federal funding.  Often these funds
are the only way NGOs can do their work. So by reinstating ‘Mexico City’ you
tie their hands and they have to pull back on critical healthcare
services.

I am sick to my stomach knowing what this means to poor woman globally.
Sick. I cry not only for our country but for women all around the world
who are now impacted by these conservative policies.
It’s not just our rights here in the US at risk but the rights of women
globally, particularly the disadvantaged.

On a cold dark night 150-200 people showed up to demonstrate in Patchogue at the occasion where Zeldin was being honored as Man-of-the-year.  Lots of energy, lots of chants and signs. Watch this video posted about 7 PM Jan 28th.

No Support for Struggling Veterans

Trump has sworn allegiance and devotion to veterans but has never actually shown it.  Now he has eliminated the Joining Forces page. Joining Forces was a program developed by Michelle Obama and Jill Biden that sought to help veterans find careers, help their spouses more easily move their careers between states, and expand wellness services.

Our congressman Zeldin also claims devotion to veterans.  Where is his outrage at Trumps actions?
